Abstract
In recent years, there has been an increase in various clinical and morphological forms of chronic pancreatitis with the peculiarities of its clinical manifestations. The most difficult in diagnosis, treatment and rehabilitation are patients with virsungolithiasis, in which concretions - virsungolites are formed in the ductal system of the pancreas due to a violation of the outflow of its secretions. Endoscopic retrograde cholangiopancreaticography (ERCP) is one of the «gold» standards in the diagnosis and treatment of pathological conditions of the pancreatobiliary system. We used a direct pancreatic duct imaging system using the SpyGlassDSIIendosystem from BostonScientific, USA, which significantly complemented the diagnostic and endotherapeutic capabilities of ERCP. The SpyGlassDSII system is successfully used for visual identification of various intracurrent neoplasms, assessment of pancreatic strictures with the possibility of biopsy, for visualization and removal of concretions in its ducts using methods of their destruction, such as laser and/or electrohydraulic lithotripsy. The advantages of performing virsungoscopy during 11 diagnostic procedures using this system are evaluated. The use of the direct imaging system «SpyGlassDSII» significantly expanded the possibilities of ERCP, which was reflected in a smoother course of the postmanipulation period, a decrease in the duration of hospitalization.

Abstract
The purpose of the study is to evaluate the possibility of using endoscopic tunnel techniques for the treatment of esophageal diverticula. Materials and methods. In the period from May 2020 to June 2024, diverticulotomy was performed by endoscopic tunnel method in six patients aged 55 to 78 years (four women, two men). Result. Such indicators as the length of hospital stay, the extent of myotomy, the duration of surgery, the duration of follow-up after surgery, the number of complications, and preliminary treatment results were evaluated. Conclusion. Endoscopic tunnel diverticulotomy in the treatment of patients with pulsed diverticula of the esophagus is a highly effective and low-traumatic operation. The evolution in surgical and innovative intra-organ treatment of patients with the above pathology indicates that the fundamental principle of intervention is an adequately performed myotomy.

Abstract
Objective: to evaluate the efficacy and safety of argon plasma coagulation for endoscopic hemostasis in Mallory-Weiss syndrome. Materials and methods: on the basis of a clinical observation of a patient admitted to the emergency department of the State Budgetary Healthcare Institution of the Moscow Region of the Children’s Central City Hospital with complaints of vomiting of scarlet blood, dark stools, weakness, dizziness. The patient underwent emergency EGDS. Results: According to the EGDS data, a linear mucosal rupture was detected in the lower third of the esophagus along the posterior wall with a transition to the stomach cardia along the lesser curvature, about 8-9 cm long (5 cm in the esophagus, 3-4 cm in the stomach), up to 2 cm deep. -3 mm, from the rupture there is a flow of scarlet blood of medium intensity, in the bottom of the defect, fibers of the submucosal and muscular layers were determined. Endoscopic hemostasis was performed: argon plasma coagulation of the rupture of the mucous membrane of the esophagus and stomach. Against the background of endoscopic treatment by argon plasma coagulation in a patient with a long rupture, hemostasis was achieved and surgical treatment was avoided, which could aggravate the condition and lengthen the treatment time. Conclusions: Argon plasma coagulation as a non-contact method of hemostasis has a limited penetration depth and is safer and more effective than other (contact) electrocoagulation methods, where, as a rule, it is difficult to control the depth of exposure. The use of this method improves the efficiency of endoscopic hemostasis, reduces economic costs, and reduces the patient’s stay in bed.

Abstract
The article discusses the problems of diagnosing Helicobacter pylori (H.pylori) using a rapid urease test (RUT). Features of H.pylori colonization and persistence are highlighted, which affect the results of RUT and determine the preferential use of biopsy specimens for the study, rather than gastric mucus aspirate, the localization of biopsy collection and their optimal number and the factors influencing the urease activity of H.pylori are indicated. Based on the described properties, the main causes of erroneous results and methods for preventing false negative and false positive results are given, the basic rules for working with RUT are formulated to obtain optimal results.

Abstract
This article presents notes from the practice of doctors of the Stavropol Regional Clinical Consultative and Diagnostic Center in the diagnosis of autoimmune gastritis. The authors noted that in the presence of endoscopic and morphological signs of autoimmune gastritis in a group of patients, there is no serological confirmation of the diagnosis.

Abstract
In the fight against colorectal cancer, there is a growing demand for more endoscopic procedures to remove colonic epithelial neoplasia. It is relevant and possible to carry out such interventions without prolonged hospitalization. The results of 330 endoscopic operations performed in the endoscopic department at a level III institution of outpatient care for the adult population of the Moscow region are presented. The method removing neoplasia was determined by the available equipment and recommendations. 1500 colon neoplasia were removed, no intraoperative complications were noted, the frequency of postoperative complications as bleeding was 0.6%.

Abstract
ISince laryngeal cancer currently occupies a leading position in frequency of occurrence among all tumors of the upper respiratory tract of modern humans, the importance of its detection at an early stage of development cannot be overestimated. Existing diagnostic methods using rigid endoscopes, including direct and indirect laryngoscopy, have significant limitations in a number of patients. The use of modern video endoscopic technology allows diagnosing precancerous lesions and laryngeal cancer at an early stage of the tumor process, which significantly expands the possibilities of minimally invasive organ-preserving treatment.

Abstract
In 2020, 2.2 million new cases of lung cancer were reported. The main form of lung cancer is non-small cell carcinoma (85%). Non-small cell lung cancer has 3 subtypes: adenocarcinoma, large cell and squamous cell carcinoma. Squamous cell lung cancer occurs in 20-40% of all lung cancers. Squamous cell lung cancer is associated with an unfavorable outcome. 5-year survival rate not exceeding 18%.

Abstract
In 2013, telemedicine began to penetrate widely and actively into the activities of the medical community. Telerehabilitation acts as an alternative within the framework of considering and conducting traditional rehabilitation or as an additional therapeutic tool, which allows you to achieve the set goal of recovery faster and more effectively. During the period 2022-2023, 1,318 people participated in the telerehabilitation program: 712 patients who suffered pneumonia caused by the new coronavirus infection COVID-19, 606 patients with chronic obstructive pulmonary disease. During the course, not a single patient was hospitalized for an exacerbation of the disease or for a clinical deterioration of the condition. At the same time, there was a significant improvement in the calculated indicators, which confirmed the relevance of this area of medicine.

Abstract
The article is based on the data and experience of the Blokhin National Medical Research Center of Oncology of the Ministry of Health of the Russian Federation, obtained during endoscopic practice. Currently, the number of pediatric patients requiring installation of a percutaneous endoscopic gastrostomy is significantly increasing, due to the increase in treatment regimens and the expansion of the range of indications for its installation. PEG combines endoscopic and surgical stages. The nurse must know the difference and differences of each stage, as well as have all the basic and advanced knowledge about all indications, contraindications and any complications during and after surgery. Compliance by the nurse with all the rules of asepsis and antisepsis, knowledge of SANPIN, is the foundation of her professionalism. Interest in the process will undoubtedly influence the quality of medical care provided. The nurse must study and know all the instruments, consumables and additional materials for performing endoscopic surgery. Her clear actions and orientation in what is happening guarantees the speed and success of the manipulation. The purpose of this work will be to emphasize the importance of professional development for nurses. To achieve the goal, we use all facets of a nurse’s work: knowledge of the concept of «sterility», orientation in all stages of manipulation from preparing the room to its complete routine cleaning after the intervention, all theoretical knowledge about the reasons for installing a gastrostomy tube and free orientation in any types of complications, etc. All of the above guarantees that the operating room endoscopic nurse is interested in the process, which will undoubtedly affect the quality and convenience of the doctor’s work, and subsequently the patient’s quality of life. The main goal of the entire operating team is to improve the patient’s quality of life. Understanding the meaning of all surgical endoscopic interventions obliges the nurse to constantly improve their skills and engage in self-education.

Abstract
Accumulated many years of experience in creating workrooms and functional endoscopy departments. Due to the fact that in our practice there are a lot of different regulatory documents in the field of construction: SNiP, codes of practice, practical design guidelines, as well as medical standards: SanPiN, sanitary rules, guidelines, various recommendations and orders of the Ministry of Health of the Russian Federation, there is a huge resonance on the requirements of these documents for the creation of working conditions for flexible endoscopy of a certain profile in a medical organization. There are no standards for the development of models of a specific profile of endoscopy departments due to its widespread use in various medical practices. The development of a project for a new medical organization does not in any way concern sanitary standards, while newly built modern medical organizations are checked precisely according to sanitary standards and do not in any way concern building codes. Such an imbalance leads us to the harmonization of multi-profile documents, averaging of area requirements, engineering solutions, conditions for linking the operating requirements from the manufacturer of high-tech equipment. The creation of working groups and the development of individual planning solutions allows us to take into account all the nuances for proper operation, work out the equipment according to technical and dimensional characteristics and clearly formulate the requirements for the preparation of endoscopy workrooms.
