Abstract
The use of therapeutic bronchoscopy in intensive care units is still actively studied and discussed. The emergence of new generations of endoscopes, new types of respiratory support, accumulation of experience in the prevention of cardiovascular complications along with the study of physiological mechanisms of bronchoscopy complications in the absence of unified views on the types of complications allows different interpretations of the outcomes of bronchoscopy in intensive care patients. Nevertheless, the search for predictors of complications, the strategy of accompanying therapeutic bronchoscopy, the choice of endoscopy type for bronchial lavage and determination of indisputable indications for interventional endoscopy remains an area of interest for physicians of different specialties, using bronchoscopy as a diagnostic and therapeutic method. The presented review of the literature summarizes the data and views of the recent years concerning the problem of therapeutic bronchoscopy in intensive care units.

Abstract
Analysis of the results of endoscopic diagnosis and treatment of patients with bronchial tumors. Material and methods. Endoscopic diagnosis and interventions were perfomed in 56 patients with benign - 39 (70.7%) and malignant - 17 (29.3%) bronchial tumors. There were 38 men and 18 women aged from 17 to 70 years (average age of patients 52.1 ± 14.6 years). Results. In patients with benign bronchial tumors, mechanical removal or excision by using polypectomy share was more often performed. In cases malignant tumors (10 patients) stent insertion was performed to ensure long-term maintenance of the bronchial lumen. In two cases - silicone stents were inserted; in eight cases - metal self-expanding stents were inserted. There were no complications during the intervention or in the postoperative period. Repeated interventions due to tumor recurrence were performed in 3 (5.4%) patients. Conclusion. Endoscopic interventions in patients with benign and malignant bronchial tumors can effectively and relatively safely restore and maintain airway patency. The choice of tumor removal method is multifactorial. It depends on the equipment of the medical institution and the endoscopist preference. To maintain bronchial patency in patients with malignant bronchial tumors, stenting is indicated, which avoids staged endoscopic interventions, each of which can be potentially dangerous due to possible complications.

Cylindroma is a malignant epithelial tumor with a slow asymptomatic course, occurring more often in women in young and middle age. This article will present a clinical case of a young patient with symptoms of acute respiratory failure due to a tumor of the middle third of the trachea, blocking the lumen by more than 2/3.

Abstract
Despite the large number of publications devoted to the diagnosis of laryngeal pathology, the issues of timely and early diagnosis of malignant neoplasms of this localization still remain very relevant. In 2022, the proportion of patients with laryngeal cancer in stages I-II was 40.3%, stage III - 30.3%, stage IV - 28.4%, while 13.1% were actively detected. Accordingly, stage III-IV cancer of the larynx accounts for 58.7%.

Abstract
The article examines the current model of operation theatre organization in surgical departments and justifies directions for its improvement. The main components of the current model include regulatory acts (NPA), equipment, principles of operation, and functions of participants. The proposed Directions for improving the operation theatre model in surgical departments are based on expanding the leadership and organizational functions of operating room nurses, redesigning the workflow model of the operating unit by incorporating principles of efficiency, safety, use of information technologies, creative management, and cost accounting methodology. It is also suggested to integrate modern quality management tools based on quality management standards into the operating unit workflow model.

Abstract
Purpose: To demonstrate the results of the first All-Russian Student Olympiad on Endoscopy. Materials and methods: The team of authors prepared and conducted the first All-Russian Student Olympiad on Endoscopy within the framework of the conference «Topical Issues of Endoscopy». The Olympiad was held in two stages, which allowed evaluating the theoretical knowledge and practical skills of the participants. The topic of the first All-Russian Student Olympiad was upper GI tract endoscopy. The first stage consisted of testing and a quiz. The test was devoted to general issues of endoscopy, and the quiz was devoted to bleeding from the upper GI tract and methods of endoscopic hemostasis. The second stage consisted of a practical part and answers to the judges’ questions. The practical part involved performance of video endoscopy on a biopreparation (porcine esophagus and stomach), identification of a conditional ulcer complicated by bleeding and performance of endoscopic hemostasis by clipping. When performing the task, the team spoke through all their actions. After the manipulation, each team answered 2 questions from the judges. Results: The results of the All-Russian Student Olympiad revealed three winners. 1st place: the team of the N. I. Pirogov Russian National Research Medical University (A. A. Klimenko, 3rd year student; K. A. Kuznetsova, 5th year student; A. I. Khizhnyak, 5th year student); 2nd place: the team of the St. Petersburg State Pediatric Medical University (D. K. Skalinsky, 3rd year student; A. D. Komarova, 3rd year student; I. A. Cherepakhina, 5th year student); 3rd place: the team of the V. A. Almazov National Medical Research Center FSBI of the Ministry of Health of Russia (A. D. Shelimanova, 4th year student; Yu. M. Bogdanov, 5th year student; M. V. Chukin, 4th year student). Conclusion: The event enabled the students to show curiosity, determine the attitude to the future profession, demonstrate their knowledge and manual skills in the field of endoscopy, as well as gain experience in teamwork. Evaluating the work of the novice endoscopists, the leading specialists in the field of surgery and endoscopy identify promising students, which has a positive impact on attracting young promising specialists to the profession.

Abstract
The article is dedicated to the plenum of the leaders of the UralEndo forum, held on April 26-27, 2024 in Tobolsk - the city that is rightfully the spiritual heart of the Urals and western Siberia, the cultural and historical center of Russia, where the current state tasks of primary health care development are closely intertwined with the tasks of preserving the best traditions of domestic medicine for the benefit of patients and our native country! In brief overview of the plenum events, the main topics are briefly presented, the achievements and problems of endoscopy in the huge Ural region are summarized, the main working tasks of the forum for the near future are formulated with reference to modern promising methods of endoscopic diagnosis and treatment based on advanced Russian and international experience of their introduction into broad clinical practice at all levels of medical care, starting from the primary level.
