Эозинофильный эзофагит. Новое Т2-заболевание с поражением пищевода

Цель: представить современные взгляды на эпидемиологию, этиологию, патогенез эозинофильного эзофагита (ЭоЭ). Основные положения. Эозинофильный эзофагит - хроническое воспалительное иммуно-опосредованное заболевание пищевода в основе патогенеза которого лежит предрасположенность к развитию иммунного ответа по пути активации Т-хелперов 2-го типа, экспрессирующих при контакте с экзогенными аллергенами чрезвычайно активную группу цитокинов. Высокая концентрация Т2- цитокинов в слизистой оболочке пищевода приводит к эозинофильной инфильтрации стенки пищевода на всем его протяжении (от проксимального до дистального отдела), развитию хронического воспаления в толще эпителия и собственной пластинки слизистой оболочки пищевода, развитию субэпителиального фиброза. У пациентов с ЭоЭ часто отмечаются сопутствующие Т2-ассоциированные воспалительные заболевания (бронхиальная астма, ринит, конъюнктивит, поллиноз и др.). Своевременная диагностика и адекватное лечение эозинофильного эзофагита позволяет предотвратить развитие стриктур и других осложнений заболевания.
