Оптимизировать эндоскопическую диагностику различных видов железистой метаплазии и дисплазии слизистой оболочки пищевода. Материал. В исследование вошло131 пациентов в возрасте от 18 до 84 лет с подтверждённым диагнозом - ГЭРБ, осложнённая развитием железистой метаплазии слизистой оболочки дистального отдела пищевода. Эндоскопические методики: Эндоскопическое исследование верхних отделов желудочно-кишечного тракта выполнялось на современной видеоэндоскопической системе EVIS EXERA III (Olympus, Япония) с использованием эндоскопов высокого разрешения. На первом этапе диагностической программы всем пациентам выполнен детальный осмотр пищевода при эзофагогастродуоденоскопии (ЭГДС) в белом свете с высоким разрешением. Затем, с целью выявления признаков дисплазии, нами использованы уточняющие эндоскопические методики: виртуальной и реальной хромоэндоскопией, и разработанные на их основе классификации BING (The Barrett International NBI Group) и PREDICT (Portsmouth acetic acid classification). Завершающим этапом диагностической программы был, забор биопсии слизистой оболочки метаплазированного сегмента по Сиэтлскому протоколу, требующему увеличения числа фрагментов по мере удлинения сегмента метаплазии при выполнении биопсии «вслепую». Полученный при биопсии материал окрашен гематоксилином и эозином, проведена ШИК-реакция в сочетании с альциановым синим по стандартной методике. Использование классификаций «BING» и «PREDICT», обеспечили маркировку компрометированных участков, позволяя прицельно выполнять забор материала для гистологического исследования. Заключение. Проведенная работа продемонстрировала наибольшую чувствительность, специфичность и точность методик «PREDICT», BING в диагностике железистой метаплазии и дисплазии у пациентов с осложненным течением ГЭРБ. Немаловажно и то, что использование инновационных эндоскопических классификаций «BING» и «PREDICT», обеспечили маркировку компрометированных участков, позволяя прицельно выполнять забор биопсийного материала. А также позволяет резко ограничить количество биоптатов, по сравнению с их необоснованно большим количеством по протоколу Сиэтла, тем самым повышая диагностическую эффективность в выявлении дисплазии при вариабельной протяженности сегмента метаплазии.</p></abstract><trans-abstract xml:lang="en"><p>Aim. Analysis of the results of using laparoscopic technologies in the city clinical hospital named after N.I. Pirogov in Orenburg Materials and methods. A retrospective analysis of all elective of emergency laparoscopic surgeries performed in the city clinical hospital named after N.I. Pirogov in Orenburg for 2020-2024 years. Results. During the period 2020-2024, 3,541 laparoscopic surgeries were performed, of which 1240 were elective (35.1%), and 2,301 were emergency (64.9%). Laparoscopic operations accounted for 67.3% of all performed surgical procedures. Emergency operations accounted for the majority of the interventions. There is an annual increase in the percentage of laparoscopic surgeries performed, and new techniques and equipment are being explored, such as bariatric and anti-reflux surgery. Conclusion. Laparoscopy in general surgery has proven to be an advantage over open surgery in planned and emergency operations.
